Forms, Templates & Online Legal Document Resources

Karen maintains a personal database of commonly requested forms and document templates that may be available to clients upon request. In addition, many forms and documents people need for notarization are also available directly from provincial and government websites. In many cases, clients can download these forms themselves and then book an appointment to have the required signature witnessed, sworn, commissioned, or notarized. Official sources include government sites for immigration, passport, education funding, court, and ServiceOntario-related forms.

Examples of forms clients may often find online include:

  • Invitation letters
  • OSAP affidavits
  • Statutory declarations, including common-law declarations and declarations in lieu of guarantor
  • ServiceOntario-related forms, such as certain vehicle, registration, and identification-related declarations
  • Immigration, refugee, visitor visa, and citizenship forms
  • Passport support forms and statutory declarations
  • Court affidavits and affidavits of service
  • Proof of life and identity-related forms
  • Travel consent letters for children
  • Powers of Attorney and related supporting documents

In addition to government forms, there are also Canadian online legal document platforms that allow people to prepare certain documents themselves, depending on their circumstances.
